Autistic Voices Form

SOS invites autistic adults who interact with SOS (such as participants of community event attendees, adult former clients, autistic employees who voluntarily choose to participate, and other autistic community members) to complete this survey in order to give input or recommendations for our organization

SOS Corporate will maintain documentation of the input received and leadership’s response. Recommendations will be considered by the appropriate clinical or administrative leader. The organization will document whether significant recommendations resulted in an organizational change or, when appropriate, why a recommendation was not implemented.

Autistic input is advisory. Final clinical, administrative, operational, legal, and business decisions remain with SOS leadership and ownership
